Student Information and Parent/Guardian Permissions Form	Click on the text in the box on the left to download the Student Information and Parent/Guardian Permissions Form.  The church office must have a completed, current copy of this form for each minor child who participates in church activities, where the child's parent/guardian is not present.
	2011 Vacation Bible School program which will be June 27 – July 1,  5:30 – 7:30 PM. All young people who participate must have a current Student Information and Parent/Guaridan Permissions Form on file with the church.    Stay Posted for 2012  VBS   on   July 9 - 13, 2012
State Park Permission Form	Click on the text in the box on the left to download the State Park Permission Form.  The church office must have a completed, current copy of this form for each child (K - 5 grade) who participates in fall Sunday School at Ludington State Park.
