Student Information and Parent/Guardian Permissions Form	Click on the text in the box on the left to download the Student Information and Parent/Guardian Permissions Form.  The church office must have a completed, current copy of this form for each minor child who participates in church activities, where the child's parent/guardian is not present.
Vacation Bible School Flyer	Click on the text in the box on the left to download information about "Baobab Blast" our 2010 Vacation Bible School program which will be July 19 – July 23,  5:30 – 7:30 PM. All young people who participate must have a current Student Information and Parent/Guaridan Permissions Form on file with the church.
State Park Permission Form	Click on the text in the box on the left to download the State Park Permission Form.  The church office must have a completed, current copy of this form for each child (K - 5 grade) who participates in fall Sunday School at Ludington State Park.
